Magandang araw po sa inyo, Ako po ay humihingi ng tulong para sa asking asawa na may sakit na Chronic Kidney Disease stage 4. Nangangailangn po na mapalitan ang kanyang fistula dahil ayon sa kanyang doctor ito ay hindi na gumagana. Kailagan pong makabitan sya ng fistula as soon as possible po kase hindi po sya mada-dialysis sabi ng doctor. Base po sa NKTI kailangan pong magbayad ng 70,000 para makabitan sya ng bagong fistula. Ako po ay walang trabao at ang aking mga anak ay sumusweldo ng minimum po lamang. Sana po ay matulugan nyo kami, Maraming salamat po,

Your Request You asked for Financial assistance sa pagpapakabit ng Fistula para sa dialysis ng aking asawa. Response to Request Please find the attached herein document on the coverage of financial assistance under the PCSO Medical Assistance Program with the respective documentary requirements. Further, we wish to inform you that the application for assistance shall depend upon the location of the health facility where the patient is being managed/ treated. For facilities within Metro Manila, assistance is processed through online application at the PCSO Official Website. Please go to , find the E-Services tab and click on the NCR Online Application for Medical Assistance. For those in the provinces, either proceed directly to the servicing PCSO Branch Office or coordinate first with the Medical Social Services or Billing Department of the facility or Malasakit Center within the facility, if applicable, to inquire on the specific process, as procedures vary between Branch Offices. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to .
