Good day po. Ako po si Michael labay Monib 35 years old. Sa mga namumuno po ng sangay na gobyerno ako po ay humihingi ng financial assistance sa tatay ko na si Dibaasun Monib 71 years old nasa hospital ngaun naka confine dahil sa sakit na severe covid19. Ang aking tatay po ay naka confine po ngaun pang pang 3rd day nya na po nka confine sa private hospital at hndi po namin kakayanin ang malaking bayarin sa hospital. July 19, 2021 Dear Michael Monib, Greetings! Thank you for your request dated Jul 18, 2021 02:32:09 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Financial assistance. Response to your request We wish to inform you that the coverage of financial assistance under the PCSO Medical Assistance Program is for hospital confinement, chemotherapy and dialysis requests. Application for assistance shall depend upon the location of the health facility where the patient is being managed/treated. For facilities within Metro Manila, assistance is processed through online application at the PCSO Official Website. Please go to , find the E-Services tab and click on the NCR Online Application for Medical Assistance. For those in the provinces, either proceed directly to the servicing PCSO Branch Office or coordinate first with the Medical Social Services or Billing Department of the facility, if applicable, to inquire on the specific process, as procedures vary between Branch Offices. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to . Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
