to get financial assistance because of covid 19 I was not able to report to work until 31st of January therefore for almost month I don't have source of income I have I years old kid and I'm renting a house for my family I'm the who working for us for for the time frame that I start to do self quarantine I don't have source of income I see Online Dole providing financial assistance to those been affected of covid 19 try to message on this website hope you will receive it get your attention I don't know were I can send all documents for the proof.this is my personal email [email protected]

March 1, 2022 Dear Gelo Peruelo, Greetings! Thank you for your request dated Feb 23, 2022 01:19:03 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for file financial assistance due to covid 19. Response to Request Have you filed EC Sickness Claim from SSS/GSIS? If you are an employee who suffered from a work-connected COVID, pls file your EC sickness claim at the SSS branch nearest you(for private sector employees); or at the GSIS (for public sector employees). The forms are available at the SSS/GSIS/ECC website. https://ecc.gov.ph/downloads/ EC Claims Requirements: 1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19. 2. RT-PCR or Swab Test Result (Positive) 3. Medical Certificate or Home Quarantine Clearance 4. 2 Valid IDs Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report) If you already have an approved EC sickness claim at SSS/GSIS, you can then file an EC cash assistance claim online at the ECC via: https://cashassistance.ecc.gov.ph from Monday to Friday, 8AM to 5PM. The EC CA application form is available in our website / FB page and the requirements are as follows: 1. Proof of Approved EC Claim Voucher from SSS/GSIS 2.
