Santiago, DPWH-FOI Officer, ([email protected] cc: [email protected]) with your company/school logo as its letterhead, duly signed by you and your supervisor/professor and a copy of your company/school identification card. We will proceed once above-mentioned requirements are provided. During the period of us awaiting your reply, your request will be put on hold.
