This refers to your request dated 07 March 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.  
You asked for information on enrollment information and transcript of records, or any information available on the following: Maximo Altaminro Sr., Domingo Altamirano and Gil Altamirano.

Since you did not provide Form F which needs your signature and should be administered under oath as required under the UP FOI Manual, we regret that we are constrained to DENY your request.

Article III Section 7 of the 1987 Constitution, which is the basis for the requirement under the UP FOI Manual, states:
