Hello. This is regarding the Endowment Fund Program of PCSO. One of the requirements for hospitals to be able to avail the said program is the Hospital Budgetary Requirement. Hence, may I request for a form/format or sample of the said requirement? We will be using this for submission of the above program. Thank you so much
