Hi! I would like to know the data for A. The number of human remains embalmed B. The name of the Embalmer Period of January 1, 2017 - December 31, 2017 For the following municipalities of Surigao del sur 1. Carrascal 2. Cantilan 3. Madrid 4. Carmen 5. Lanuza This is also in reference to #DOH-539816901964 which is the the whole Surigao del Sur. Example would be: Name of Embalmer - Number of Cadavers Embalmed in a monthly basis. Thanks

March 22, 2018 Dear Francisco Casio Mata, Greetings! Thank you for your request dated Mar 05, 2018 02:16:30 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Embalmer's Data. Response to Request Your FOI request is approved. However, please be informed that we can only provide you the data on the number of registered embalmer per municipality. (Carrascal-0, Cantilan-1, Madrid-2, Carmen-0, Lanuza-1) As of this moment, the DOH Regional Office CARAGA does not keep data on the number of cadavers embalmed, while the data on the names of the embalmers cannot be released due to privacy concerns. However, if you want to push through with this request, the Committee of Undertakers and Embalmers advised that you send them a formal letter stating the full title of your study, together with its rationale and objectives. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Undersecretary Lilibeth C. David, Chair of Data Governance Oversight Committee / DOH FOI Central Appeals and Review Committee, at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
