Hi sir ma'am. Pls. send me GSIS ECC form because I want to request financial assistance from ECC office because I got Covid 19 last August 11, 2021 and got healed after 14 days of home quarantine with proof swab test. I'll appreciate it much if you could grant my request.

Relative to your query, kindly find attached list of basic requirements for EC Covid related case for government employees. You may file for Employees Compensation claim by complying the documentary requirements . Claim may be submitted thru drop box at nearest GSIS Office.
