HI. I WOULD LIKE TO FOLLOW UP ON MY REQUEST of CERTIFICATE FOR MY APPLICATION ON SSS UNEMPLOYMENT BENEFITS. DOLE ALREADY ISSUED ME A CERTIFICATE BUT THE DATE INDICATED ON THE CERTIFICATE IS NOT SAME IN MY AFFIDAVIT, THATS WHY MY APPLICATION IS REJECTED BY SSS. I ALSO REQUESTED THIS ON POEA ([email protected]) BUT UNTIL NOW, NO STATUS WAS GIVEN. HOPING FOR YOUR IMMEDIATE ACTION AND RESPONSE

March 5, 2021 Dear LEODENILO REYNOSO, Greetings! Thank you for your request dated Mar 01, 2021 08:59:39 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for DOLE CERTIFICATION. If you are an OFW, please send your request to POEA thru this email address: WSB @poea.gov.ph the following requirements: (1)One (1) valid ID (preferably passport or any government issued ID and (2) Copy of the Notice of Termination issued by the employer or duly notarized Affidavit of Termination of Employment in the absence of Notice of Termination. The Affidavit of Termination should be supported by proof of the OFW's arrival to the Philippines such as a stamp of arrival by the Bureau of Immigration in the OFW's passport.
