November 30, 2020 Dear Mr. Michelle Dimapilis, Greetings! Thank you for your request dated Nov 28, 2020 03:48:27 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA records show that you applied online on August 25, 2020 and evaluated by OWWA-RWO 4A as PENDING for submission of Proof of Loss of Employment on account of COVID-19. You repplied on November 27 and 30, 2020. SHOULD YOU FAIL TO SUBMIT THE REQUIREMENT NEEDED, THE PROCESSING OF DOLE-AKAP application will not proceed. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter.
