January 29, 2021 Dear Ms. MARINEL MERANO, Greetings! Thank you for your request dated Jan 05, 2021 09:13:59 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. The change for name of claimant must be requested by the OFW to the DOLE Regional Office that released the DOLE-AKAP financial benefit. May mga hihingin po sa inyo na mga documents tulad ng Authorization or SPA and proof of relationship. It is the DOLE and NOT OWWA, that processed the financial benefit, Hence, DOLE Regional Office po ang may karapatan magdecision sa usapang change of name of claimant. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
