Thank you for your request dated Jan 22, 2021 01:22:16 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA Verification System shows that you applied online on December 20, 2020 and was evaluated by OWWA-RWO 4A as APPROVED on 01/13/2021. Your financial benefit was RELEASED by DOLE Regional Office IV-B on 01/13/2021 with tracking no. VKZ-15-26278-ZAW. With respect to the change of claimant you or your legally authorized representative must inform the DOLE Regional Office IV-A for the proper procedure. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected].
