April 12, 2021 Dear Ms. FATIMA IRISH MENDOZA, Greetings! Thank you for your request dated Apr 06, 2021 10:08:31 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA Verification System shows that you applied online on December 19, 2020 and was endorsed by OWWA Regional Welfare Office 4A to DOLE Regional Office IV-A for further verification since worker cannot provide documentary evidence of LOSS OF EMPLOYMENT ON ACCOUNT OF COVID-19. Follow-up with DOLE Regional Office IV-A at Calamba City. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected].
