Hi.I would like to inquire how can i change the name of claimant of my AKAP assistance.I received text message from Palawan pawnshop,however i am already here in the quarantine facility.Palawan pawnshop will not release the money assistance to my authorize representative even if we presented an authorization letter.Please help me with the procedure or who/where can we call to about this matter.thank you

March 26, 2021 Dear Mr. Jefferson Agustin, Greetings! Thank you for your request dated Mar 25, 2021 05:35:38 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA Verification System shows that you applied online on December 29, 2020 and was evaluated by OWWA-RWO 4A as APPROVED on 03/15/2021. Your documents were forwarded to DOLE Regional Office IVA for the release of your DOLE-AKAP financial benefit. With respect to the release for your DOLE-AKAP Benefit, You or your representative will have to coordinate with DOLE Regional Office IVA so they can make the necessary documents for a change of claimant. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
