Dear OWWA R4-A, I, Mineirdo Pacunla Reyes, Jr., writing this letter to request in your good office to change the claimant/receiver of my approved DOLE AKAP CASH ASSISTANCE for the reason that the suffix (Jr.) in my name doesn't appear in your system and it prevents me from claiming the cash. Thank you for your request dated Mar 04, 2021 01:22:56 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. Your documents were forwarded to DOLE Regional Office IV-A for the release of your financial benefit. Follow-up with DOLE Regional OFFICE IV-A.
