Goodmorning sir/mam, We already received the control # of dole akap assictance but i can not claim it as i am now in saudi Arabia i would like to request the change name to Rose Ann Macaraeg, my live in patner that we can claim the akap assictance . March 12, 2021 Dear Mr. Edgar Balderama, Greetings! Thank you for your request dated Mar 11, 2021 08:47:12 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-AKAP Application. OWWA Verification System shows that you applied online on January 9, 2021 and was evaluated and APPROVED by OWWA RWO 1. Your documents were forwarded to DOLE Regional Office I and released your DOLE-AKAP benefits to a remittance center on 02/25/2021. Any change of claimant must be coordinated with DOLE Regional Office I and NOT with OWWA. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
