Hi good day! I am Lovely Fate Jala from DOH RO-IX Regulation, Licensing and Enforcement Division. I would like to request the following documents to serve as guidelines and reference to the implementation of the assessment tool for licensing hospitals (ANNEX K-1) 1. DOH Department Memorandum 2017-0061 2. Department Circular No. 70 s. 1996 "National Archives of the Philippines" 3. Administrative Order 2008-0009 "Policies and procedure on reporting notifiable diseases" 4. Administrative Order 2004-0168 "National Policy on Health Emergency and Disaster" Hoping for a response.
