You asked for DO 174-17.
The following are the requirements for request for exemption for registration under DO 174-17.
1. Business Permit under DTI.
2. Mayor's Permit
3. SEC Registration and Articles of Incorporation/Partnership, indicating the company primary purpose (if applicable)
4. BIR Registration
5. Copy of contract between parties or Purchase Order
6. Copy of PCAB License for construction-related businesses
7. Request letter addressed to ASEC. MA. TERESITA S. CUCUECO, MD, CESO III
You may submit this to DOLE BWC at 3/F DOLE Central Office, Muralla ST. cor Gen. Luna St., Intramuros Manila or submit a soft copy of the documents to [email protected]. Please don't forget to include contact details of your company in your request letter.
