May 17, 2021 Dear Rosario Mateo, Greetings! Thank you for your request dated May 11, 2021 01:30:07 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Moreover, please attach the following requirements for the renewal of your Discharge Permit: 1. Duly signed and notarized application form (can be downloaded using the link: https://llda.gov.ph/discharge-permit-dp-renewal/) 2. Updated certificate of interconnection issued by Maynilad 3. Three months water bill Filing of application is manually done in our office, Monday to Friday, 8 am to 4 pm. Evaluation and assessment will be made after submission of documents and processing takes 20 days after payment.
