Good day, I would like to request information from your good office, rest assured, the information will be treated with much confidentiality. I badly need the information to complete my thesis as part of my degree on Architecture. I'm willing to send identification if you wish to.
The following is the information I will request, but not limited to.
1. Disability Statistics of AFP
*Number of disabled personnel from 2015-2020 (or what year is available)
*Number and/or list of Disabilities among AFP personnel
2. Other available information you can give about disabilities of personnel.

I refer to my letter of Dec 02, 2020 08:08:54 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Disability statistics among disabled AFP (e.g. list of disabilities, number of disabled, year, etc).
As per the AFP FOI Manual, please submit a fully accomplished AFP FOI Request Form (attached) along with two valid government-issued IDs, security clearances (NBI, PNP, Barangay), and a letter from your school endorsing your research. You may find a copy of the AFP FOI Manual here:

Dear Maze Ann,
I refer to my letter of Dec 14, 2020 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Disability statistics among disabled AFP (e.g. list of disabilities, number of disabled, year, etc). In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Mar 08, 2021.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
