Hi, Sir,Madam,
as a Recipient/Beneficiary of Dole integrated livelihood program (DILP), grant by Department of labor and employment (DOLE) Region 9 Zamboanga city Field office to Zamboanga pepsi employees worker's union (ZPEWU) In the year of march, 2016. And in behalf of other beneficiary
That i would like to request for a copy furnished of DILP approved project proposal submitted by the ZPEWU in the year of 2016, Together with the list of names of all 106 beneficiaries and the enrollment of 106 beneficiary to Group personal accident insurance that was encoded at Dole DKIS or in GSIS,

Dear Mr. Mallari,
Greetings!
This is in response to the letter regarding the request for the documents of the livelihood project titled Rice Trading with 106 beneficiaries amounting to 209,000.00 which was approved last March 15, 2016.
Our record shows that all 2016 dated files were already disposed in accordance to the rules of Republic Act 9470 otherwise known as the "National Archives of the Philippines Act'.
In line with this, this office can no longer provide the requested documents.
