Good day. I would like to request Columbary financial assistance for my common-law spouse who died on August 11, 2021, due to covid19. My common-law spouse suffered acute schizophrenia-like Psychotic disorder due to Covid19. We've been undergoing quarantine to Quezon City quarantine facility last August 03, 2021, but due to an unexpected situation, my partner suffered depression to Psychotic disorder and the facility decided to transfer her to National Center-for Mental Health to applied appropriate medication for both illnesses (covid19 and mental health) but died last August 11, 2021.

Respond to ypur request While our aim is to provide information whenever possible, in this instance this Office does not have financial assistance for Columbary. We wish to inform you that the coverage of financial assistance under the PCSO Medical Assistance Program is for hospital confinement, chemotherapy and dialysis requests. Application for assistance shall depend upon the location of the health facility where the patient is being managed/treated. For facilities within Metro Manila, assistance is processed through online application at the PCSO Official Website. Please go to, find the E-Services tab and click on the NCR Online Application for Medical Assistance. For the Medical Assistance Program requirements, kindly go to this link https://www.pcso.gov.ph/ProgramsAndServices/CAD/MedicalAccessProgram.aspx. For those in the provinces, either proceed directly to the servicing PCSO Branch Office or coordinate first with the Medical Social Services or Billing Department of the facility, if applicable, to inquire on the specific process, as procedures vary between Branch Offices.
