Magandang araw po. Nais ko pong humingi ng tulong pinansyal para ipambayad sa ospital (Pagamutan ng Dasmariñas) kung saan namatay ang aking byenan na lalaki na si Pemy F. Fadero, 59 taong gulang, sa sakit na Chronic Kidney Disease, kasalukuyan po kaming nka- promisory note sa ospital, sobrang laki po ng inabot na bayarin, (97,000) at wala po kaming kakayanan na ito po ay mabayaran. Nawa po kami ay inyong matulungan. Maraming Salamat po.

January 6, 2022 Dear Hope Joy Gabrieles, Greetings! Thank you for your request dated Jan 05, 2022 01:25:53 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Died due to Chonic Kidney Disesase. Response to your request We wish to inform you that the coverage of financial assistance under the PCSO Medical Assistance Program is for hospital confinement, chemotherapy and dialysis requests. Application for assistance shall depend upon the location of the health facility where the patient is being managed/treated. For facilities within Metro Manila, assistance is processed through online application at the PCSO Official Website. Please go to , find the E-Services tab and click on the NCR Online Application for Medical Assistance. For the Medical Assistance Program requirements, kindly go to this link https://www.pcso.gov.ph/ProgramsAndServices/CAD/MedicalAccessProgram.aspx. For chemotherapy, post kidney and liver transplant/ hemophilia medicines and epoetin injection under the PCSO Medical Access Program, please find the attached herein new procedures for the availment of medicines for outpatients under the Medical Access Program in NCR. For those in the provinces, either proceed directly to the servicing PCSO Branch Office or coordinate first with the Medical Social Services or Billing Department of the facility, if applicable, to inquire on the specific process, as procedures vary between Branch Offices.
