Good day po gusto ko langpo humingi ng tulong meron po sigmoid adenocarcinoma ( stage 3 ) ang MR. Ko. Naoperahan po cya sa veterans memorial sa qc. Akala po namin yung professional fee po ma covered ng malasakit cash po pala babayaran. Humihingi po ako ng tulong mg promisory note lang kami sa hospital di po namin kaya bayaran ang 111,000 na naiwan namin. Sa ngayon po ng chechemo therapy po ang asawa ko at di rin po alam kung paano dahil wala kami trabaho sa ngayin dahil sa kalagayan ng Mr. Ko. Sana po matulungan niyo kami.maraming

Your Request You asked for Dennis Gamboa. Response to Request Attached herewith are documentary requirements for each type of assistance under the PCSO Medical Access Program. We wish to inform you that the application for assistance shall depend upon the location of the hospital/health facility where the patient is being managed/ treated and if the hospital/facility has a Malasakit Center. Patient confined or being treated in hospitals/facilities with Malasakit Centers should submit their application for assistance thru the Malasakit Centers, while patients being treated in hospitals/facilities with no Malasakit Centers should apply for assistance with the nearest branch offices or thru Online Application for hospitals/facilities in the National Capital Region. You can find the list of Malasakit Centers nationwide through our website at www.pcso.gov.ph. Look for the Programs and Services tab and click Medical Access Program under CAD Programs and Services. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to .
