Response to Request
Have you filed EC Sickness Claim from SSS/GSIS?
If you are an employee who suffered from a work-connected COVID, pls file your EC sickness claim at the SSS branch nearest you(for private sector employees); or at the GSIS (for public sector employees).
The forms are available at the SSS/GSIS/ECC website.

EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs
Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)
If you already have an approved EC sickness claim at SSS/GSIS, you can then file an EC cash assistance claim online at the ECC via:
https://cashassistance.ecc.gov.ph
from Monday to Friday, 8AM to 5PM.
The EC CA application form is available in our website / FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
