Good afternoon, Kindly requesting that my record be updated/properly encoded on VIMS so that i may generate my vaxcert document for international travel. I have already forfeited 1 ticket and have rebooked this second 3 times. I have been at the mercy of my LGU. It is very difficult to rely on them for a timely and proper response which is why i am reaching out on this platform. Bday: april 2, 1991 Where: coron palawan 1st dose: october 27, 2021 2nd dose: november 17, 2021 (on LGU vax id printed oct 17. This is also what was encoded) Hoping for your kind consideration

Response to your request Please use this process if: Details of vaccination record have been incorrectly encoded, or You would like to update your information Click “Update record” to begin the process of updating. You will have to upload a photo of your vaccination card and government-issued ID and enter your email address and/or contact number. Your request will be forwarded to the LGU where you received your vaccination so they can validate and rectify through your email address or contact number once your request is completed and you may try requesting again your VaxCertPH through the website. VaxCertPH is a portal and mobile app developed by the DICT for the Department of Health (DOH) for the issuance of digital vaccination certificates for vaccinated individuals in the country.
