Hi, I would like to request for the standard process of correcting a wrong birthplace in the Form 137 (Elementary) of a previous student. Is it possible to resolve the correction of birthplace in school level? What are the requirements needed? The student has already graduated from the school last 2017. And the parent is only requesting for the Form 137 now. The residence address was written in the space provided for the place of birth. I am hoping for your swift assistance on this matter. Both records should be reconciled and should be decided as to what name to follow.
