Good afternoon po. Maginquire po sana paano makakakuha ng copy ng sac form? Kinuha po kasi un kalahati nung form sa kanya sa barangay kaso kailangan pala un para s application online. Andun daw kasi un barcode po at SAC #. Hindi na po siya inaasikaso ngayon sa barangay dahil priority daw yung hindi pa nabibigyan. Inatake po siya sakit sa puso at hika nung una pila nya s pagkuha ng form at muntik na dalhin ospital pero hindi siya pumayag. Pero hiningi po sa kanya un kalahati. Ngayon po, ang kailangan lang po niya is ung kopya ng kalahati ng form para makapagapply online. Sana matulungan nyo
