I would like to request for copies of the case file on Mr. Teddy Eugene Lear in connection with his deportation from the Philippines on April 27, 2015. Specifically, I would like to request for copies of the following:
1. Deportation order issued against Mr. Lear in SDO NO. SBM/BOC-14-151;
2. Decision of the BOC in connection with the above case;
3. Charge sheet against Mr. Lear in connection with the above deportation case.

In relation thereto, kindly note of Section 12, par. a of Procedure of Access, People’s Freedom of Information Act of 2012 which states that: “The request shall state …..XXXX….. the reason for request of the information.
Further, Section 6.1, par 1.1 of Bureau of Immigration Manual on Freedom of Information states that:
• “the request must be in writing;
• the request shall state the name and contact number information of the requesting party, as well as provide valid proof of identification or authorization; and

I am requesting for the case record of Mr. Teddy Eugene Lear for the purpose of filing a petition for the removal or lifting of the blacklist against him issued by the Philippine Bureau of Immigration. The petiton I previously filed was denied, because I don’t have the ocrrect and complete information about the case, hence this request.

I refer to my letter of Aug 10, 2018 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Copy of decision in SDO NO. SBM/BOC-14-151 involving TEDDY EUGENE LEAR, born 11/24/43, US Citizen.. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Nov 02, 2018.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
