Greetings! I, Mr. Kennedy C. Cuya, a CHED-SIKAP Grantee, would like to request on the final template of the Continuing Eligibility Evaluation Form stated in the CMO No. 16, series of 2022, Appendix B, Table 6. The said form is requested to facilitate our scholarship allowance. We were informed that the said form was not yet been finalized and there's no definite date on its finalization. This would have an effect on our claims, enrollment procedures, and other concerns. To document this request as a follow-up, I respectfully request the finalized aforementioned form. Dios Mabalos.
