Hi Ma'am/Sir,
Good Day ! 
I would like to request regarding the cash assistance for covid 19.While I was at work Oct 23, 2020 I noticed that I was losing my sense of smell and I decided to go had my check-up the day  after but while I was asking our Brgy Center how I could make an appointment for my check up,they advised me to take Swabtest first.Oct 27 was my Swab Schedule,Oct 28 I was tested positive for covid 19,Oct 29 -Nov 10 i was taken to the facility, Nov 10 - Nov 19 under home quarantine and Nov 20 i finally got back to work. I hope you can help me and thank you very much.
