Hi! I would like to ask how to change surname in school records? I am planning to continue my study in college. Since kinder up to college I used "Morga" as my surname. I lost my copy of birth certificate which my mother gave it to. So I decided to get a new one last year and when it delivered to me and check the birth certificate my surname is "Soriano" not "Morga". So what should I do to change it?

Response to Request Please be informed that the inconsistency of a person's records is not only an issue at the Department of Education (DepEd), but a legal issue as well. Both records should be reconciled and should be decided as to what date of birth to follow.
