Hi! I would like to know what Last Name I should use for booking an appointment for renewal of my passport. I have an expired passport bearing my mother's maiden name, however, I have been able to change my last name and use my father's. Now, I want to book an appointment with DFA but I don't know what last name to use because my old passport does not match my birth certificate now. Please shed some light on this concern. Thank you.
