October 17, 2022 Dear Brian Toledo, Greetings! Thank you for your request dated Oct 15, 2022 06:31:01 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Change claimant for dole akap. Response to Request Good day. Kindly send the following requirements/attachments of your request for change claimant to [email protected] / [email protected]. 1. Authorization Letter of OFW signed by the OFW (Write the contact number of the new claimant) 2. New flight details of the OFW 3. 2 valid IDs of the new claimant w/ birthdate 4. Proof of relationship 5. 2nd & 3rd Page of OFW Passport (make sure the 3rd page is signed) The subject email should be, FOR CHANGE OF CLAIMANT <>. After sending the documents, kindly wait for the email/text from OWWA that your request is in process. Don't go to USSC if your request is not yet confirmed. You should wait for the text message from the remittance center.
