Response to Request This is to inform you of the process and requirements by which to apply for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION for SSS issued by DOLE-NCR. An employee - Local or Overseas Filipino Worker (OFW) who was involuntarily separated by their employer may file their application for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION at the DOLE-NCR website thru the e-filing services tab by clicking this link: https://docs.google.com/forms/d/e/1FAIpQLSfhNqxP-qEiWAZUmjuTK8ayWHRxFcEfZeJn3kLmv2zDSrAiYw/viewform The requirements for the issuance of the certificate of involuntary separation for uploading online are as follows: a. valid email address (google account) b. at least one (1) valid I.D.
