Response to your request You may apply for Driver’s License Certification / Driver’s License History at the License Section, LTO Central Office, East Avenue, Quezon City. Kindly send your authorized representative, a copy of your driver’s license, authorization letter and a copy of a valid government issued ID of your representative, a corresponding fee of Php 100.00 is charge for the said certification.
