Good morning! I would like to request for a Certification from your office that (1.) the Land Transportation- Oroquieta City have been accepting Medical Certificates from an unlicensed Physician, particularly a Certain Optometrist with a surname "Pausanos" who have been holding Office beside the LTO itself in Lower lamac, Oroquieta City. Further, I would also like to obtain a Certification (2.) that Medical Certificates from the unlicensed Physician received by LTO Oroquieta City were transmitted and received by the LTO Regional Office and were accorded due course by the Regional Office
