Good day Maam / Sir My Name is Liezel Galicia Diaz, I would like to request Certificate of Involuntary resign because im already separated to my employer last Dec.23 2021. due to company financial problem because of national pandemic COVID 19. Ill try to access E-service for my request but no available for Bulacan areas. I pass my application on SSS website and the certificate is one of the requirements to process my request. January 27, 2022 Dear Liezel Diaz, Thank you for your request dated Jan 26, 2022 01:19:06 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for Certificate of Involuntary Separation. Please be advised that pursuant to the Department Circular No 01. Series 2019-Section IV- Venue of Filing of Application for DOLE Certification stated that "employees who are involuntary separated may file their application for DOLE certification at the DOLE Field Office of jurisdiction". Separated employees may apply via walk-in but due to the pandemic, affected employees are highly encouraged to apply online if the service is available.
