Response to Request We regret to inform you that DOLE Region 5 does not have an e-service for processing requests for Certificate of Involuntary Separation. Instead, please submit the following: 1. Duly Filled-Up Application Form; 2. SSS Transaction Number; 3. Photocopy of one (1) valid ID with photo and signature; 4. Copy of Notice of Termination of Employment Issued by the Employer; or Duly Notarized Affidavit of Termination of Employment in the absence of Notice of Termination of Employment You may choose to submit these requirements personally or via the email address of Camarines Sur Provincial Office - [email protected] Attached is a copy of the application form, and a copy of SSS Circular No. 2019-011 for your reference. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the DOLE Secretary at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
