I made a request last time for my COE, thank you very much since the DOH (Western Visayas) contacted me thru my email, however they cannot grant the COE on my maiden name - that's understandable.
I have already filled the form they have forwarded on my email, however, up until now I haven't received my COE. I also another colleague that has been waiting since June for her COE. I was wondering if FOI can help again to process our COEs quicker. Thank you!

2. Kindly indicate the name of your colleague who is requesting for a COE. What office was she stationed and the duration of her employment?
