Hi Sir good day!
The document that I am requesting will be use as one of our evidence/proof in the pending Falsification of Documents filed against my client before METC br. 29, Manila. The document involved is a Surety Bond released by the Monarch Insurance Company, Inc. way back 2008 which my client used as a surety in his Appeal in a Labor Case in NLRC Region IV-A. We only got copy of the said Certificate of Authority of Monarch Insurance Company, Inc. from the NLRC, the Court want us to secure certified copy from the Insurance Commission. Thank you!
