May I request from your good office a copy of the census of hemodialysis patients in North Cotabato (specifically in Midsayap, Pigcawayan, Libongan, Aleosan, Kabuntalan, North Kabuntalan, and Datu Piang) purposely to be used as a reference for my Feasibility Study for Establishment for a Hemodialysis Center in North Cotabato. Looking for a positive response. Thank you!

October 14, 2021 Dear Diana Libunao, Thank you for your request dated Oct 13, 2021 10:21:04 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for Census of hemodialysis patients in North Cotabato. Our latest data is 2016. Based on this report, there were 685 patients on dialysis in Region 12.
