Thank you for your request dated Sep 02, 2022 03:46:40 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for CAV. Response to Request REQUEST FOR CAV - To request for CAV, you may personally visit the Registrar’s Office from Monday to Friday 8:00AM to 4:00PM and provide a photocopy of your Transcript of Records and Diploma for authentication purposes. Present the copy of your TOR and Diploma at the Registrar’s window and pay Php 50.00 at the Cashier’s Office.
