Good day! I am a public school teacher. I was afflicted with Covid, and I am presently under quarantine. I was informed that I could claim for cash assistance due to this incident. I would like ask for a form so i could file an application. Thank you

Relative to your query, kindly find attached list of basic requirements for EC Covid related case for government employees. You may file for Employees Compensation claim by complying the documentary requirements. Claim may be submitted thru drop box at nearest GSIS Office. Thank you
