To whom it may concern: I am asking for financial assistant for my father. who is currently undergoing of treatment for Covid 19 in Global Medical Center of Laguna. as a result of unexpected and significant medical expenses. if you could help us what ever you can provide, we would be most appreciative.

September 29, 2021 Dear Marvin Elazegui, I refer to my letter of Sep 28, 2021 11:18:43 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Cash assistance for medication and hospital bills of patient with Covid 19 and Pneumonia. Pursuant to Section 4 (1.1) of the PCSO People’s Freedom of Information Manual, kindly provide us a valid proof of identification. You may upload in your eFOi portal account a valid proof of identification. During the period of us awaiting your reply, your request will be put on hold.
