Hi, I would like to ask, how can I have the Certificate of my booster shot? Because it’s needed to the country that I’m headed. I’m planning going overseas next month and have my schedule for booster shot on 11th Feb 2022. I would like to know how can i get the certificate. Thank you!

Response to your request While our aim is to provide information whenever possible, in this instance this Office could not provide your request for issuance of Booster Shot Certificate or vaccination certificate. However, you may wish to contact Department of Health (DOH) doh.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at their website.
