Hello,
My husband had covidDecember 2020. He received his SSS Sickness Claim and EC Sickness Claim. However he hasn't received the 30,000. We are referring to Board Resolution No. 21-04-14 - ECC lists Covid-19 as ‘compensable disease’: DOLE ( see link : https://www.pna.gov.ph/articles/1138347 ) -
"Bello said an employee affected by Covid-19 will be receiving PHP30,000.
“It will become a compensable illness. The usual compensation on work-related illness is PHP10,000 but if it’s Covid-19, PHP30,000 under this ECC resolution,” he said. “There is no distinction (mild or severe), if it’"

As indicated please see the following information:
If you are an employee who suffered from a work-connected COVID, pls file your EC sickness claim at the SSS branch nearest you(for private sector employees); or at the GSIS (for public sector employees).
The forms are available at the SSS/GSIS/ECC website.
EC Claims Requirements:
1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19.
2. RT-PCR or Swab Test Result (Positive)
3. Medical Certificate or Home Quarantine Clearance
4. 2 Valid IDs
Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report)
If you already have an approved EC sickness claim at SSS/GSIS, you can then file an EC cash assistance claim at the ECC.
The EC CA application form is available in our FB page and the requirements are as follows:
1. Proof of Approved EC Claim Voucher from SSS/GSIS
2. RT-PCR or Swab Test Result showing POSITIVE for CoVid-19, issued by a DOH accredited testing center / Medical Certificate / Home Quarantine Clearance issued by LGU Health Office/BHERT
3. 2 Valid IDs
Your right to request a review
