Hello po. I read about your responses on stolen phone blocking requests and see the need to submit an Affidavit of Ownership and Loss with Undertaking (NTC form), however, I can’t seem to locate the document/form online and there is no NTC branch in our area. I would like to report and block my lost phone, Vivo V9, and I can provide you the barcode and S/N as proof of ownership. As for the Affidavit of Ownership and Loss with Undertaking (NTC form), may I ask po for a copy or link to download and fill up the file? Looking forward for your response po. Thank you!
