Hi good day, I would like to request about me and my son Birth certificate,My name is Loveliness M. Natividad my Son is Shan Michael Camaisa,My father request a form that he and her wife was the parents without my conscent..Please help us for my son future.Thank u Godbless

May 18, 2020 Dear Loveliness Natividad, Greetings! Thank you for your request dated May 18, 2020 05:03:01 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You asked for Birth certificate ng anak ko. Your FOI request is approved. Please be informed that our PSA Serbilis Outlets and online application services located in areas identified to be modified extended on enhanced community quarantine shall remain closed to the transacting public. Meanwhile, CRS Outlets situated in areas classified under General Community Quarantine (GCQ) shall re-open and resume providing frontline services for the processing of PSA-issued documents beginning 04 May 2020 subject to the GCQ rules from the Office of the President. Moreover, CRS Outlets in these areas shall also begin to accept applications for PSA-issued documents thru Batch Requests System (BREQS) filed at Local Government Units (LGUs) and SM Business Centers.
