Hi! I would like to request a copy of my BIR 2316 Form (Certificate of Compensation Payment/Tax Withheld) for the year 2021.

This is because it is one of the requirements needed by for my credit card application. I already requested a copy of my BIR 2316 from my employer, but the copy I received was not signed.
